  • Answers telephone inquiries promptly and provides information regarding available services to the caller.
  • Provides linkage immediately to individuals discharged from PESS and hospitals to obtain prompt services.
  • Enters accurate and complete data into the electronic health record.
  • Schedule appointments and aid in all aspects of the intake process.
  • Triage calls and schedules the client accordingly.
  • Obtains insurance and authorization information from caller.
  • Provides support and reassurance to individuals and their families to reduce concerns associated with the intake process and refers callers to outside agencies and services when appropriate using a warm hand-off if possible.
  • Maintains standards of confidentiality and Routes calls to appropriate staff person based on needs.

Summary:
  • The Intake Triage Specialist is responsible for conducting a point of entry interview with individuals, their families or other referral sources. When complete they will schedule individuals for initial intakes with a clinician and provide service delivery linkages to referral agencies (PESS and hospitals) when indicated. The incumbent enters all intake information into an electronic heath record and consults with clinical staff regarding a provisional level of care recommendation.
Essential Functions:

  • Enters accurate and complete data into the electronic health record.
  • Schedule appointments and aid in all aspects of the intake process.
  • Triage calls and schedules the client accordingly.
  • Obtains insurance and authorization information from caller.
  • Requests medical records and other clinical information, when available, from professional referral sources prior to evaluation.
  • Provides support and reassurance to individuals and their families to reduce concerns associated with the intake process and refers callers to outside agencies and services when appropriate using a warm hand-off if possible.
  • Maintains standards of confidentiality and Routes calls to appropriate staff person based on needs.
  • Performs other duties as assigned.
Essential Qulifications:
  • Bachelor's degree in psychology, Social Work, or related field
  • 1+ years on Mental and/or Medical Health Care setting
  • Understanding of trauma-informed, wellness and recovery, person-centered delivery of care, and biopsychosocial development through the life span
  • Advanced oral and written communication skills
  • Ability to multitask and prioritize competing demands.
  • Demonstrated ability to work effectively with behavioral health clients, referral sources, and community agencies.
  • Must have excellent telephone engagement and customer service skills.
  • Must be computer literate and able to navigate between several programs.
  • Valid NJ Driver's License in good standing
Preferred Qualifications:
  • Basic counseling skills
  • Bilingual ability (Spanish/English)
  • Experience/sensitivity working with culturally diverse populations.
  • Lived experience in mental health and/or substance use disorders and in recovery

  • Ability to work within a team environment as well as work independently.
  • Ability to communicate and interact with County employees and officials, as well as members of organizations doing business in County facilities, vendors, contractors, and residents
  • Ability to comprehend, interpret and apply policies and/or other official documents.
  • Skill in the operation of office equipment, including but not limited to computers, printers, copiers, calculators, and other job specific equipment.
  • Ability to accurately prepare and maintain files, records, reports, and logs.
